A report by the Australian Public Service Commission revealed that 12 current and former officials breached the public service code of conduct 97 times during the unlawful robodebt welfare recovery scheme. Key figures, Kathryn Campbell and Renee Leon, were responsible for 25 breaches. The scheme, which wrongfully pursued debts from nearly 400,000 individuals, caused significant distress and was linked to several suicides. Four current officials received sanctions, but others faced no penalties.
