2023 Quebec police sergeant death ruled avoidable due to systemic failures in mental health care and law enforcement communication.
A Quebec coroner's report determined that the March 27, 2023, deaths of provincial police sergeant Maureen Breau and the mentally ill man who stabbed her, Isaac Brouillard Lessard, were avoidable. Coroner Géhane Kamel cited systemic failures in assessing and supervising Brouillard Lessard, who had a history of mental illness. The report included 38 recommendations aimed at improving communication between mental health services and law enforcement.
