62-year-old telecom subcontractor John McCann died after falling 24 meters down a ventilation shaft in Dublin, Ireland, onto a fragile floor surface identified as a hazard 15 months prior to his death. The fragile surface, consisting of insulation and fire-resistant material, had a "do not enter" sign deemed inadequate by the Health and Safety Authority. Ove Arup, the company that owns the building, has since conducted a review of all its buildings globally and issued an improvement notice to increase safety measures.
