2022 Virginia bus crash caused by fatigued truck driver with falsified records from Triton Logistics, violating federal hours-of-service regulations; NTSB recommends improvements and enhanced oversight.
The National Transportation Safety Board (NTSB) has determined that a fatal bus crash in Virginia in December 2022, which resulted in three deaths, was caused by a fatigued truck driver working for Triton Logistics Inc., a company that allowed its drivers to log excessive hours. The truck driver had falsified his driving records, which enabled him to exceed federal hours-of-service regulations. The NTSB has recommended that Triton Logistics improve its verification of driver records and implement a robust fatigue management program, as well as calling for better state and federal oversight.
