56-year-old Craig Tracey died from massive internal bleeding after a vein was accidentally pierced during a hernia surgery at Beacon Hospital in Dublin. Despite the rare risk not being previously identified, surgeon Professor John Reynolds performed the surgery. Tracey suffered a sudden, unforeseeable complication and passed away despite medical efforts, leading the coroner to record a verdict of death by misadventure.
