Health NZ Southern (formerly Southern DHB) violated the Code of Health and Disability Services Consumers' Rights in providing care to a woman who required spinal surgeries. Breaches included inadequate information provision and obtaining informed consent before administering steroids without discussion. The Deputy Health and Disability Commissioner criticized the organization's poor record-keeping and recommended an apology, review of neurosurgery staff training on informed consent, and an improvement in record-keeping standards.
