36-year-old mother, Jade Ellis, experienced cardiac arrest with chest pains, but was wrongly diagnosed as having a trapped nerve and sent home with paracetamol. The Cwm Taf Morgannwg University Local Health Board admitted liability for breach of duty and causation, stating that if adequate tests had been conducted, Jade would not have been discharged and likely would have survived. Jade's family received an apology and compensation in July.
