19-year-old wildland firefighter Devyn Gale died due to inadequate training, supervision, and unsafe work procedures in British Columbia's wildfire operations.

19-year-old wildland firefighter Devyn Gale was killed in British Columbia near Revelstoke last summer when a burning cedar tree fell on her during a wildfire operation. WorkSafeBC's incident investigation report found that ineffective hazard management, inadequate training, supervision and orientation of young workers, unsafe work procedures, and a normalization of risk culture within the BC Wildfire Service were factors in Gale's tragic death. Premier David Eby directed the BC Wildfire Service to implement changes based on the report's findings to ensure worker safety and adequate training.
