Oceania Care Centres breached the Code of Health and Disability Services Consumers' Rights by failing to provide appropriate care to a vulnerable 90-year-old woman, who later died. The woman, assessed as high-risk, experienced a fall during her stay, leading to pain and drowsiness. Whareama Care Centre, under Oceania, also violated Right 4(4) for not minimizing harm and optimizing quality of life.
