A glitch in the outdated and fragmented 111 emergency call system caused a delay in dispatching an ambulance to a fatal house fire in Auckland's Hillpark suburb in May. The system's channel, called InterCAD, experienced random outages, increasing in frequency from 2021 to early 2024. Comms staff did not immediately realize the system had failed, adding to the ambulance response delay.
