2018 Wānaka triple-fatal helicopter crash caused by rear door opening, overalls striking tail rotor.

A new report by the Transport Accident Investigation Commission (TAIC) indicates that the unexpected opening of a rear door in a helicopter near Wānaka Airport led to overalls flying out and striking the tail rotor, causing a triple-fatal crash in 2018. The accident killed two Department of Conservation workers and pilot Nick Wallis. The report also highlights the need for clearer instructions and adherence to maintenance manuals to prevent similar incidents.
