A New Zealand pharmacy breached the Code of Health and Disability Services Consumers' Rights by incorrectly dispensing a fast-release insulin pen, NovoMix 30, instead of the woman's usual slow-release pen, Lantus Solostar. The woman suffered sweating, shaking, dizziness, blurred vision, and fainting episodes due to the error. The Deputy Health and Disability Commissioner found that the pharmacy breached the woman's right to appropriate standard services.
