Christchurch woman Toni Shields mistakenly received a cancer diagnosis due to a file mix-up, leading her to plan her own funeral. Health New Zealand acknowledged the error, which occurred when incorrect images were loaded into her file on May 22. The organization notified her GP of the issue on May 29, and staff also spoke directly with Shields, who had spent six days believing she had cancer.
