Melbourne's Casey Hospital fined $160,000.

Monash Health was fined $160,000 by the Melbourne County Court for failing to ensure patient safety after a patient suicide in 2015. The patient was admitted at high risk of self-harm and required regular welfare checks. Despite an identified risk in the visitors' toilet, no action was taken. The court found it was reasonably practicable for Monash Health to reduce or eliminate the risk of harm.
