Darren Coleman claims Ireland's mental health system failed his wife, who had post-natal depression and psychosis, leading to her and their baby's deaths.

Darren Coleman, whose wife killed their baby son and herself, has said Ireland's mental health system is broken. Coleman spoke in the High Court as he settled an action over the care received by his wife, Nicola Keane, who had post-natal depression and psychosis. The HSE has admitted liability and apologised for the care received by Ms Keane, who later took her own life and that of her baby.
