NSW Government provides $300,000 to Community Industry Group (CI Group) for a project aimed at easing transition of older patients from hospital to aged care. The project will implement a volunteer coordinator program and fund research to better understand & address the needs of long-stay patients. It aims to reduce delays in discharging older people from hospital and develop regional health and aged care plans.
