69-year-old mother Olwen Collier died after falling 12ft into a pub's cellar while decorating for her daughter's surprise birthday party. The owners of the Stag and Pheasant pub in Carmel, south Wales, were spared jail after pleading guilty to failing to discharge general health and safety duty to a person other than an employee. Judge Paul Thomas KC described the pub's arrangements as "a fatal accident waiting to happen" due to dim lighting, poor signage, and unclear directions leading to the unlocked cellar.
