A 76-year-old woman, Elizabeth Murphy, collapsed and died after experiencing anaphylactic shock following an antibiotic prescribed by her doctor. University Hospital Waterford has offered "sincerest apologies" to the family and has settled the High Court action for €400,000. The hospital had not included in Ms. Murphy's discharge papers her previous anaphylactic reaction to the antibiotic in the hospital, leading to her tragic death.
