61-year-old man, Andrew Holland, suffered a psychotic episode for weeks after being prescribed a new drug, prednisolone, at Manchester Royal Eye Hospital. Holland was not warned about the side-effects of the steroid, which led to hallucinations of bears and burglars. The Parliamentary and Health Service Ombudsman found that Holland was not fully informed before receiving the prescription.
