63-year-old sex offender Reginald Roach died from self-inflicted genital amputation in North Wales in November 2022.
63-year-old sex offender Reginald Alan Roach bled to death after amputating his own genitals in a field near Bangor, North Wales, in November 2022. Roach had been released from prison six days prior and was staying at Ty Newydd Approved Premises, where staff provided effective care. An investigation by the Prisons and Probation Ombudsman found that Ty Newydd should have contacted Roach's family within two days of his death, which was ruled a suicide due to shock and haemorrhaging.
