The Transport Accident Investigation Commission (TAIC) adds rail worker safety to its watchlist, following a near-death incident involving track workers on Wellington's Johnsonville train line. Miscommunication, assumptions, and incorrect procedures led to the train being authorised to drive through a worksite. TAIC calls for KiwiRail to improve safety procedures and ensure workers are well-rested and in a fit state to work.
