The report found no indicators or precipitating events that could have been acted upon to prevent the tragedy, but highlighted issues such as discrepancies in allocated and actual time for parole board members, and suggested that the Correctional Service of Canada should consider reinstating the community corrections liaison officer program which was eliminated in 2015.
