A woman who consented to a hysterectomy due to pelvic organ prolapse was given a surgical mesh treatment instead without her informed consent. The Health and Disability Commissioner found that the doctor failed to provide adequate information to the woman, breaching the Code of Health and Disability Services Consumers' Rights. The woman was informed of the change in procedure just before entering the operating theatre, after she had already been premedicated and signed a consent form for the surgical mesh implant.
