Aneurin Bevan University Health Board in Wales has apologised to a grieving family after a hospital released the wrong body for burial. The mix-up occurred at The Grange hospital in Cwmbran and was caused by "human error". The health board has implemented additional safeguards and requested a follow-up audit by the Human Tissue Authority. This is the second such incident at the hospital in a short period of time.
