A pharmacy breached health & disability rights after a 4-year-old boy was hospitalized due to a dispensing error with hay fever medicine. The boy received antipsychotic haloperidol instead of loratadine. The pharmacy has since implemented new processes, acquired tablet-counting and blister-packaging machines, increased staffing levels, and is conducting an audit of 20 randomly selected dispensed liquid medications.
